<p><strong>一、什么是痔疮</strong></p><p>凡肛门内外突起的柔软肿物都叫痔疮。因为直肠下端或肛管存在丰富的静脉丛，如果在一处或数处发生扩张或曲张，即成为痔。狭义的痔疮是位于肛管直肠黏膜下端及肛管皮肤下的静脉淤血、曲张，而后形成的血管团，是肛肠疾病中的一种。</p><p style="text-align: center;"><img height="160" center;">混合痔实例图</p><p><strong>二、痔疮的症状</strong></p><p>痔疮在初发时，症状往往不明显，但随着痔核逐渐增大，症状亦会逐渐加重。临床常见的症状有以下几点：</p><p>1．便血：微痛性、间歇性便后有鲜红色血是其特点，也是内痔或混合痔早期常见症状，患者常诉说“大便后鲜血从肛门一滴一滴地往下流”或“便纸上都是鲜血”。</p><p center;">痔疮便血</p><p>2．痔核脱出：常是晚期症状，多先有便血，后有脱垂，由于痔核体积增大，排便时受到粪便的挤压，使其逐渐与肌层分离而脱出肛外。较初仅在排便时脱出，便后能自行还纳。症状较重者，脱出后需用手推回，或卧床休息，方能复位。症状更严重者，除排便时脱出外，凡用力、行走、咳嗽、喷嚏、下蹲等，都可能脱出。</p><p>3．疼痛：单纯内痔，一般无疼痛，有时仅感觉肛门部坠胀或排便困难。如发炎肿胀者，痔内有血栓形成或嵌顿，则有疼痛；如脱出未及时复位，则疼痛加重；如发生嵌顿，有溃烂坏死，引起肛缘周围皮肤发炎水肿，则疼痛剧烈，患者坐卧不安。</p><p>4．粘液流出：直肠黏膜受痔的刺激，引起分泌物增多；晚期内痔，因肛门括约肌松弛，常有分泌物由肛门流出。轻者大便时流出，重者不排便时也自然流出。内痔脱出时，分泌物将更多。</p><p>5．瘙痒：因分泌物或脱出痔核的刺激，使肛门周围皮肤终日潮湿，导致湿疹和瘙痒的发生。</p><p><strong>三、痔疮的原因</strong></p><p>目前的医学研究认为痔疮的发生原因，有以下几个方面的因素：</p><p>1．解剖学原因：人在站立或坐位时，肛门直肠位于下部，由于重力、脏器和粪便的压迫，静脉向上回流颇受障碍，容易扩张屈曲，形成痔疮。</p><p>2．遗传关系：静脉壁先天性薄弱，抗力减低，不能耐受血管内压力，因而逐渐扩张。</p><p>3．职业关系：某些职业的人群需要久站或久坐或负重远行，影响静脉回流，使盆腔内血流缓慢和腹内脏器充血，引起痔静脉过度充盈，静脉壁张力下降，血管容易瘀血扩张。又因运动不足，肠蠕动减少，粪便下行迟缓，或习惯性便秘，导致痔疮的发生。</p><p>4．局部刺激和饮食不节：肛门部受冷、受热、便秘、腹泻、过量饮酒和多吃辛辣食物，都可刺激肛门和直肠，使痔静脉丛充血，影响静脉血液回流，以致静脉壁抵抗力下降。</p><p>5．肛门静脉压力增高：因肝硬化、肝充血和心脏功能代偿不全等，均可使肛门静脉充血，压力增高，影响直肠静脉血液回流。</p><p>6．腹内压力增加：因腹内肿瘤、子宫肿瘤、卵巢肿瘤、前列腺肥大、妊娠、饮食过饱或蹲厕过久等，都可使腹内压增加，妨碍静脉的血液回流。</p><p>7．肛门部感染：痔静脉丛先因急、慢性感染发炎，静脉壁弹性组织逐渐纤维化而变弱，抵抗力不足。而致扩大曲张，加上其他原因，使静脉曲张逐渐加重，生成痔块。</p><p><strong>四、痔疮的危害</strong></p><p>痔疮同其他疾病一样，早期治疗难度低、好得快。如果贻误病情，错过较好治疗时机，不但治疗难度大、恢复慢，甚至会对身体造成不可恢复的损害。痔疮会造成哪些危害呢：</p><p>1．肛门功能失常。久病不愈，一方面导致肛门挛缩，排便困难；另一方面侵犯括约肌，易导致肛门失禁。</p><p>2．导致败血症、毒血症、脓毒血症。肛门脓肿、肛门感染一旦形成，产生出血症状，细菌、毒素、脓栓就容易侵入血液引发败血症，毒血症、浓毒血症。</p><p>3．引发妇科疾病。由于肛门和阴道接近，痔疮出血或发炎往往会导致细菌大量繁殖，引发妇科炎症。</p><p>4．易致植物神经紊乱。可引起腰部、骺骨、股后部位疼痛，还可引起泌尿生殖系统反射性功能紊乱。</p><p>5．皮肤湿疹。由于痔块脱出导致括约肌松弛，黏液流出肛门外刺激周围皮肤，易导致瘙痒及皮肤湿疹。</p><p>6．掩盖直肠癌。有关资料显示，我国直肠癌的误诊率在70%-88.57%之间。痔疮与直肠癌较突出的相似症状就是便血，不少人把肠癌的出血当作是痔疮出血，结果延误治疗时机，等到去就医时往往已是中晚期。其实，在现有医疗条件下，许多发生在结肠或直肠的恶性肿瘤，如果能够早诊断、早治疗，奏效机会还是比较大的。</p><p><strong>五、痔疮怎么治疗</strong></p><p>现代医学认为，治疗痔疮的目的是消除或减轻其主要症状，而非通过手术将痔切除达到痔的治好，没有必要甚至错误的手术治疗可导致肛门功能障碍，因此治疗痔疮应尽量采取非手术的方法。</p><p>然而，重度痔、急性痔，无论是病理解剖还是生理功能已经不再具有可逆性，选择手术治疗是必要的，但手术中不可任意扩大手术范围，应尽可能地保存肛垫组织，保护肛门功能。</p><p>痔的临床表现复杂，病情较长，不同时期，不同类型，应选择不同的治疗方法，不能盲目采用其中一种方法代替其他所有方法，应该强调个体化的治疗原则。下面列出几例医博医院对于不同症状的痔疮患者手术前后的对比图，供患者参考：</p><p center;">传统手术与医博PPH手术前后对比图</p><p><strong>六、痔疮怎么预防</strong></p><p>痔疮是人类特有的疾病，它的发生、发展有其规律性，只要预防得当，可以减慢减少，或者减少痔疮的发生或减轻症状。一般而言，痔疮的预防可以从以下几个方面进行，如：加强体育锻炼，保持心情舒畅，注意饮食调理，调整劳逸起居，促进正常排便等。</p><p>1．保持大便通畅。养成每天及时大便的习惯。早晨起床可喝一杯凉开水，能促进排便。大便时不宜看书报，否则将忽略便意，日久造成粪便停滞，形成干硬粪块，不易排出。不宜过久地蹲厕努挣，久蹲久坐，可导致痔静脉曲张出血。</p><p>2．注意肛门清洁。保持肛门清洁，常用温水清洗，勤换内裤。节制性生活，注意性生活的清洁卫生。会阴部分泌物较多者，应便后或睡前用温水清洗肛门。勤换内裤，清洗的内裤应在阳光下晒干。</p><p>3．治疗原发疾病。应及时治疗肛周疾患及全身性疾患，可减少痔的发生。如蛲虫病、滴虫病、肛周湿疹、便秘、腹泻、白带过多、高血压症、心脏病、肺气肿、糖尿病等。</p><p>4．注意饮食卫生。在饮食方面，要注意荤素搭配，粗、细粮调剂合适。饮食宜清淡、润肠通便，宜食含纤维素多的新鲜蔬菜和水果。刺激性食物会刺激肛门黏膜皮肤而局部充血、灼痛，加重痔疮发病，应忌食。另外痔疮患者还应忌烟酒，避免暴饮暴食。</p><p>5．坚持体育锻炼。从事久坐、久立工作的人，工作1-2小时后，应适当走动10-15分钟。平时可选择跑步、打太极拳、健身操、工间操等活动项目，能促进血液循环和肠蠕动，减少盆腔充血和痔疮发生。没事的时候可多做提肛运动，对预防痔疮有很好的效果。</p>
